Geriatric Care Management and Alzheimer’s Care Management services for local and long distance caregiving solutions. Our Geriatric Care Navigation Program™ is a progressive, affordable and empowering approach to traditional geriatric care management, and provides a professional family for caregivers and families to turn. at any point in their caregiving journey.
Caregiver Coaching
Caregivers have the opportunity to work with a professional geriatric care manager, registered nurse consultant, or social worker, to help them manage and cope with their own emotions and individual challenges that accompany carrying for someone with Alzheimer’s disease. Caregivers can also receive immediate crisis care and crisis support in our offices or in their own home.
Making Moments Matter
Making Moments Matter™ is a daily, person-centered program helps you reconnect and create the best possible day for your loved one. Perfect for those unwilling or unable to attend adult day care, as well as those needing socialization and appropriate cognitive stimulation. Caregivers also learn how to reconnect to their loved one and communicate in a way that is better understood by the person with Alzheimer’s disease or dementia.
