Most families want to do everything they can to keep the person with Alzheimer’s disease at home, and every day the Care Navigators at the Alzheimer’s Care Resource Center work with caregivers and families to try to make this possible.
But if the time comes when living at home is no longer an option, your Care Navigator will help you select the very best assisted living facility or nursing home based on your needs and the needs of your loved one.
Each Preferred Provider has successfully demonstrated their commitment to meeting the Preferred Provider standards as set forth by our Board of Directors; which includes extensive initial and ongoing training requirements for all direct care workers; as well as strict adherence to the principals and best practices, of person-centered dementia care.
Your Care Navigator will assist you during the entire selection, placement, and transitional process. She can assist with packing and moving; provide coaching and counseling services; assure all paperwork and medical records are complete; arrange for a private duty caregiver to help your loved one adjust; provide ongoing monitoring and patient advocacy; and will continue to work closely with you and your family in the days, weeks and months that follow.
