Today I want to talk to you about the geriatric care management services that we provide at the Alzheimer’s Care Resource Center.
The field of geriatric care management, although not new to professionals, is still a fairly new concept for Alzheimer’s caregivers. Especially for caregivers who are already overwhelmed with the responsibility of caring for someone with dementia.
Each week we receive a call from at least one caregiver who isn’t sure how a care manager can help them, their family and the person they care for.
Each caregiver who reaches out to the Alzheimer’s Care Resource Center has the opportunity to meet with a geriatric care manager that specializes in Alzheimer’s disease at no charge.
It is during this free visit that together, the care manager and the caregiver determine if geriatric care management services are appropriate, given their unique situation.
Some caregivers find that they need help managing the complex medical needs of their loved one, while others may need assistance in selecting an assisted living facility or nursing home or learning how to cope with changing roles and difficult behaviors.
While the Care Management services at the Alzheimer’s Care Resource Center can be quite comprehensive, many care management services are used in a more episodic way. Caregivers utilize the services of their Care Manager on and off throughout their caregiving journey, and always work with the same person.
Although not all caregivers need the services of a professional geriatric care manager, we are committed to making our care management services available, affordable, and accessible to as many caregivers in our community as possible.
