On April 1, 2014, the Alzheimer’s Care Resource Center will open two new offices in Boca Raton and Palm Beach Gardens, Florida.
Each office will provide caregivers of those with Alzheimer’s disease, dementia or other neurocognitive impairment, the opportunity to participate in Alzheimer’s support groups, attend individual counseling or counseling sessions, receive education and training and have access to a vast amount of community based resources and referrals.
For caregivers and families seeking additional assistance with complex care issues, we have expanded our Geriatric Care Navigation program. Geriatric Care Navigation is a progressive and empowering approach to traditional geriatric care management.
It provides professional expertise to help assess your individual needs; thoughtfully craft appropriate care options; empower you as a caregiver and as a family; and help you to make choices that enhance your loved ones safety and quality of life. Our registered nurse consultants, care managers and social workers will then assist you in implementing your chosen Care Navigation Plan™.
