Our Geriatric Care Navigation Program was designed as a progressive and empowering approach to traditional geriatric care management to offer a professional family for caregivers to turn. Geriatric Care Navigation helps caregivers and families feel reassured that they are never alone in facing their loved ones care needs.
Caregivers are often overwhelmed with making choices for their loved one. Coping with current care needs and trying to anticipate the future can create stress and very often fear of the future. The good news is caregivers never need to feel alone and can feel hopeful for that even in complicated situations, solutions exist to make a positive difference for the caregiver and the care receiver.
Here at the Alzheimer’s Care Resource Center™, our unique Geriatric Care Navigation “model of care” is a systems approach which views every person within the family as important and individually playing important roles as a whole system. Studies have shown that our approach is extremely beneficial to the care receiver, caregiver and their family.
Our clinicians, called Care Navigators™, are Master’s level social workers, counselors and certified geriatric care managers who provide clinical expertise and experience with the dementia journey to help assess individual needs, thoughtfully craft appropriate care options and then empower caregivers and families to make choices to enhance their loved ones safety and quality of life. Care Navigators™ often assist clients with implementing Care Navigation Plans and provide piece of mind with ongoing support to ensure a person the Alzheimer’s disease or dementia is maintaining their activities of daily living, connecting to resources and managing essential medical care such as medication management, physician appointments and maintaining appropriate nutrition and activity plans. Care Navigators™ are also available to provide vital emotional support to caregivers, care receivers and families as well as Caregiver Coaching to enhance the caregiver’s approach to communication, managing behavior challenges and meeting the caregiver’s own important self care needs.
Most of our patient’s not only suffer from Alzheimer’s disease or dementia, but have been diagnosed with other coexisting chronic conditions such as: diabetes, heart disease, arthritis, congestive heart failure (CHF), anxiety and/or depression. The Care Navigator™ directs a multidisciplinary team that ensures that all of the patient’s needs are met and partner with physicians and involved professionals to advocate and create a cohesive care team. Care Navigators™ create Care Navigation Plans that not only provide ongoing and continuous care, but that incorporate all of the medical, physical, social, psychological and spiritual needs of the patient.
