Question: How do your geriatric care managers, social workers and nurse consultants help caregivers that have a loved one with Alzheimer’s disease?
Answer: The main goal of our dementia specific geriatric care navigation program is to help caregivers, and their families, cope with the myriad of challenges that come with an Alzheimer’s/dementia diagnosis, and that often continue throughout the caregiving journey.
Recognizing that each caregiving situation is unique, the first thing that we do is to meet with you to begin to assess exactly what your needs are. As always, this visit is free. One of our nurses, geriatric care managers or social workers, will consult with you by telephone or in person, at our office – in The Gathering Place, or at a time and place most comfortable and convenient for you.
This first visit is so important. It’s an opportunity for you to get to know us and to become comfortable sharing your fears, concerns and challenges. It’s the time to have all of your questions answered, and to take the first step to receiving all of the care, support, guidance and professional advice you might need to help you navigate in your role as an Alzheimer’s caregiver.

- Behavioral problems such as: repetitiveness, sundowning, agitation, aggression, and wandering
